35 day evaluation program

Identifying and evaluating the strengths and areas of intervention of a youth and their family is invaluable. Our assessment provides families with recommendations and options made by professional, caring staff while giving immediate treatment.


 

Areas of assessment and discharge recommendations:

  1. Behavioral
  2. Psychological - a licensed psychologist specializing in child and adolescent development issues
  3. Psychiatric - an assessment by a board-certified psychiatrist
  4. Academic
  5. Family
  6. Recreational
  7. Physical - physical, dental and vision exams as appropriate

 

Optional Assessments*

  1. Cultural - completed to address cultural and spiritual strengths
  2. Psycho-sexual - completed if the need is identified
  3. Neurological - if warranted, scheduling assistance is given to determine if other conditions impact daily functioning

*Optional assessments may extend 35-Day Evaluation upon discussion and approval of the referral agent.

 

Mental Health and Chemical Dependency (MI/CD) Evaluations**
Youth struggling with mental health issues and chemical dependency concerns can receive a comprehensive MI/CD evaluation by an integrated team of professionals upon referral. Evaluation components include:

  • Rule 25 Assessment at on-set of placement with results clinically discussed prior to psychological testing
  • Six dimensions incorporated in the chemical dependency evaluation
  • Chemical dependency counselor consults with the evaluation team providing feedback and results.
  • Final report including comprehensive chemical dependency data and recommendation section
  • Medication Holiday (youth ceases use of all prescribed medications), under contracted psychiatrist supervision, to provide "clean" mental health and chemical dependency assessments.

 

Paving The Way Home
Step down services help to ensure an uncomplicated transition home, and North Homes bridges the gap between life in a structured environment versus a less restrictive home. A successful transition requires preparation by everyone (families, communities and other care partners). We facilitate and collaborate to ease the stress felt by youth and families as they prepare for change. Step down services plan and implement necessary home customizations, as well as create a network of reliable support for everyone.
